    The next date for Sports Physicals with the School Doctor is TUESDAY, MAY 24th @ 10:00 AM. 

    Submit all PPE packets to the school nurse completed with parent signatures by THURSDAY, MAY 19th. 

    Students having their own physicals with a private pediatrician must turn in their completed PPE forms by WEDNESDAY, MAY 18th.

    ***Permission and Participation forms must be completed online BEFORE you can have a physical with the school doctor.

    If your child is sick, PLEASE keep him/her home.

    Call the attendance office at 732-634-5858 x.3010 to report the absence.

    If you take your child to the doctor, please ask for a note and bring it in when returning to school.


    Emergency Cards: 

    2 new emergency cardmust be completed annually for each student and returned for use in emergency situations.

    Additionally, during the school year, 2 new emergency cards must be filled out if there is any change of address, phone number or parent place of employment.

    Students without emergency cards will not be permitted to participate in school field trips or other activities.


    Medication Policy and Permission:

    Prescription or OTC medication for students will only be given during the school day with parental and doctor's authorization.

    Required School-issued form for Medication Authorization

    All medication must be in a properly labeled prescription container and brought to the Nurses’ Office upon arrival at school.

    Under no circumstances will any other medication be allowed to be taken in school.

    Students who are prescribed self-administered medication for asthma are strongly urged to keep a backup inhaler in the Nurses’ Office for an emergency.


    Physical Exams:

    All 9th and 11th-grade students, as well as all new students at the Academy for Allied Health and Biomedical Sciences, are required to have a physical examination upon entrance to the school.

    If, for any reason, the parent or guardian wishes that the student have the examination done by the family physician, the parent must sign and check the appropriate box on the reverse side of the emergency card.


    P.E. Excuse Procedure:

    If, for any reason, a student cannot participate in the full gym program, a note must be brought from home.

    If this disability extends for more than five (5) school days, a doctor’s note must be submitted indicating the length of time you are to be excused. The note should be brought to the school nurse for recording.

    Under no circumstances are students with medical excuses allowed to participate in sports or gym activities. Students must remain where assigned unless excused by the teacher in charge.

    All doctors’ notes to be excused from or limit Physical Education participation must be renewed, in writing, each school year.
